Early Registration forms and payment will be accepted until September 20, 2004.
Early Registration fee is $225.00. This includes materials, breakfasts and lunches. Please complete the attached registration form and return with payment by September 21, 2004. After this date, the late registration fee is $300.00.
All charges must be paid in U.S. currency in the form of a personal check drawn on a U.S. bank or U.S. money order made payable to: University of Puerto Rico, Mayagüez Campus.

Cancellation Policy
Cancellations received before October 6, 2004, will be refunded in full, minus a $25.00 processing fee. Cancellations received after that date will be refunded in half (50%). All refunds will be made after the Conference is over.
