Applications Instructions
Application – Deadline Extended to November 14, 2008 ( for undergraduate scholarship )
You must submit the following documents. Incomplete applications will be disqualified and will not be considered for a scholarship. If you are an undergraduate or graduate student click on the Undergraduate and Graduate Forms below, respectively.

Application Form - Undergraduate Form, Graduate Form. Fill out the form and print when completed.

Academic Transcripts - All applicants must submit one (1) official transcript from each college and university attended for work completed prior to
fall 2008. Transcripts should be received no later than November 21, 2008. It is the responsibility of the students to make sure that the transcript is received by the established date.

Faculty Sponsorship Form – All students must be sponsored by a UPRM faculty member, who will supervise the research work conducted by the student. The sponsorship does not have to involve financial commitment. Faculty sponsors will have to fill out an evaluation/progress at the end of each academic term. It is the responsibility of the student scholarship recipient to provide the faculty sponsor with the evaluation form, and submit it to the scholarship program office in a sealed enveloped at the end of each scholarship term.

Two Letters of Recomendations – Provide two (2) letters of references using the Reference Form. One of the references may be from your faculty research sponsor. At least one of the references must be from a UPRM faculty member. It is the responsability of the students to makesure that the letters of recomendations are submitted on time.

Submit the entire application package (application form, faculty sponsorship form, and sealed letters of recommendation). The application package can be mailed or delivered physically
to the address below, but MUST be received by the application deadline.
Applications received after this deadline will not be considered.
