To comply with federal and institutional requirements, all active UPRM investigators must submit an annual FCOI disclosure prior to September 1 of each calendar year. To file an annual disclosure, complete the following steps: 1. Fill out and sign the following form: FCOI Form 2A. 2. If the answer to any of the questions in Form 2A was "Yes", proceed to fill out and sign FCOI Form 2B. 3. Scan your completed and signed documents and email them to fcoi@upr.edu. |
